Creating a form/survey


  1. Click on Add New Form
  2. Enter a name and description for your form and click create. If you would like to use an existing form as a template then select Copy an existing form and select the form that you would like to use as a template.
  3. Once created, you will need to follow the 6 steps listed to create your form.
    The form you create will not be active until you have published your changes.
  4. Once published, you can access your form under the contacts menu, via the Form URL button or by embedding it on your website with the html available under the security section.
  5. After you have collected data you can view it by creating a report under Reports > Forms.
